Description
Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, significant others, and other health team members while maintaining high standards of professional nursing. Provides high-quality care to adult patients at end of life. Coordinates all hospice services required for optimal end of life care. Night shift 1900 - 0730. Every third weekend rotation required, and every other holiday rotation required. This position will be 24 hours one week (2x12s) and 36 hours the following week (3x12s) for a total of 60 hours in a two-week scheduling period. Previous Epic experience within the last twelve (12) months required.
