Description
We are seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services to patients in the Grand Junction. The Physical Therapist will deliver skilled physical therapy services in patients' homes in accordance with physician orders, payer requirements, agency policies, and applicable state and federal regulations. This role supports patient mobility, safety, and functional independence through individualized, evidence-based care. Responsibilities include evaluations, developing care plans, skilled interventions, and documentation compliance.
