Description
As a Case Manager, you will coordinate patient care, manage treatment plans, and ensure compliance with healthcare regulations. This role requires a strong focus on patient advocacy and collaboration with multidisciplinary teams. You'll be working in a vibrant community that offers both natural beauty and a variety of activities.
