Description

As a Registered Nurse in Home Health, you will provide skilled, patient-centered care in the community. This position offers flexibility in structuring your day, manageable caseloads, and meaningful one-on-one patient care. You will perform assessments, develop care plans, and educate patients and families, all while working independently in a supportive team environment.

Requirements
  • Strong assessment and critical thinking skills
  • Clear, professional communication with patients and team members
  • Ability to work independently and manage a structured caseload
  • Valid driver''s license, reliable transportation, and auto insurance
  • EMR System: HCHB preferred
  • Home health experience preferred, but not required
  • Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license)
  • MinExperienceRequired: 0
  • Comfort with documentation and EMR systems (HCHB preferred)
