Acute care experience including ICU Nutrition support experience Preferred or nice to have: candidates with these skills will be considered first. NICU experience List typical procedures performed on unit(s): ICU rounds Nutrition support Nutrition assessments Educations Best personality Fit: Adaptable personality REQUIRED CERTIFICATIONS: RD, LD Floating Area(s): ICU, Telemetry, General floor, Woman s unit, ER Dress Code: Scrub Color/Attire - Black Scrubs Unit Guidelines/Policies: Please feel free to attach unit policies and guidelines. A nutrition assessment will be completed if a patient warrants an assessment based on the nutrition triggers and the nutrition care plan. The Registered Dietitian will develop a nutrition care plan as follows: Within 24 hours of: Physician ordered consult for nutrition support (Enteral or Parenteral Nutrition) Mechanical Ventilation/Intubation Within 72 hours of : Admission nursing screen referral with Registered Dietitian validation of moderate or high nutrition risk Initiation of specialized nutrition support (Enteral or Parenteral Nutrition) NPO/CL 3 days Pressure Ulcers (All Stages) Malnutrition Score of 2 or greater Dysphagia Obstetrical nutrition triggers (see Section K) Within 10 days of: Patients with extended lengths of stay (more than 6 days) are re-screened to assess potential increased nutrition risk during hospitalization. The RD, using clinical judgment, will classify each patient as LOW, MODERATE, or HIGH nutrition risk after assessing the patient. When a nutrition consult is ordered, an assessment, progress note, or education note will be documented in Meditech by the RD. Consults for nutrition education will be completed by the RD within 48 hours of notification and documented in the interdisciplinary patient/family education section in Meditech. Consults for diet education may include but are not limited to Coumadin, Congestive Heart Failure, diabetes, or weight loss. Nursing educates the patients on Coumadin at Northwest and Bay View and consults the RD as needed. Weekends and On call(if any) No weekends or on call General Comments No. of Positions: 1/1.
