Description
As a Hospice Nurse, you will be responsible for admitting patients, assessing their needs, and developing individualized care plans. You'll coordinate care with a multidisciplinary team and conduct follow-up visits. This role emphasizes a compassionate approach to patient and family care in a variety of locations.
