Description

Physical Therapist – Contract / Contract-to-Hire (Sacramento, CA)

Join a forward-thinking healthcare organization dedicated to helping older adults remain independent and thrive in their communities. We’re looking for a Physical Therapist to provide compassionate, evidence-based care to participants in a PACE (Program of All-Inclusive Care for the Elderly) setting.

As part of an interdisciplinary team, you’ll work with physicians, nurses, social workers, and rehabilitation specialists to create personalized treatment plans aimed at improving mobility, strength, balance, and quality of life for participants.

Position Highlights:

  • 13–26 week contract role with the potential to convert to a permanent position if it’s a great fit
  • Work in a mix of outpatient clinic, participants’ homes, and community settings
  • Supportive, mission-driven culture focused on helping older adults remain safe and independent

What You’ll Do:

  • Evaluate participants’ functional status and develop individualized treatment plans
  • Provide PT services in clinic, home, and community environments
  • Collaborate with an interdisciplinary team to promote safety and wellness
  • Document progress and adjust plans as needed
  • Educate participants and caregivers on safe movement, fall prevention, and home exercise programs

Qualifications:

  • Licensed (or license-eligible) Physical Therapist in California
  • Passion for working with older adults
  • Strong communication and teamwork skills
  • Experience in geriatrics, outpatient rehab, or home health preferred

Compensation & Perks:

  • Competitive hourly pay (commensurate with experience)
  • Full-time, Monday–Friday (no evenings or weekends)
  • Comprehensive benefits available for longer-term assignments
  • License reimbursement at the completion of the assignment if you’re not already licensed in CA
