Licensed Practical Nurse (LPN) – Home Health (PACE Program)
Location: Denver, CO 80210

MedAdventures is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our PACE Home Health Program in Aurora, CO. This is a meaningful opportunity to provide essential nursing care to older adults in their homes, helping participants maintain independence and quality of life in the community.
As an In-Home Services LPN, you’ll deliver general and skilled nursing care to participants in accordance with agency policies and applicable regulations. This is a field-based role requiring clinical visits in participant homes within the designated coverage area.

Key Responsibilities:
- Provide general nursing care and perform assigned skilled procedures per plan of care
- Supervise or administer prescribed medications and treatments in accordance with standards
- Implement primary care provider and nursing orders as needed
- Perform phlebotomy functions when necessary
- Instruct participants and caregivers on medications and treatment plans
- Observe and report participant conditions and responses to treatments
- Administer vaccinations and TB testing; document results appropriately
- Prepare clinical notes and complete required audits and documentation
- Accept and execute orders (to be co-signed by RN)
- Involve participants and families in the care planning process
- Participate in peer reviews, quality management activities, and utilization reviews
- Provide direct nursing care in participant homes
Qualifications (Required):
- Active Colorado LPN license
- Current BLS and First Aid certifications (American Heart Association or American Red Cross)
- Reliable personal transportation, valid driver’s license, and proof of auto insurance
- High School Diploma or GED
Preferred:
- 3+ years of LPN experience in a clinic, home health, or long-term care setting
- Experience working with geriatric populations
- Familiarity with medications used in elderly care
