Position Overview

We are seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. In this role, you’ll assess patients, design personalized treatment plans, and provide hands-on therapy to enhance mobility, strength, and overall function for residents.

Key Responsibilities

• Conduct patient evaluations to determine therapy needs and goals.
• Develop and implement individualized physical therapy treatment plans.
• Provide evidence-based therapeutic interventions to restore movement and relieve pain.
• Educate patients and caregivers on exercises, safety, and home programs.
• Collaborate with an interdisciplinary care team including nurses, physicians, and occupational therapists.
• Document patient progress and maintain accurate treatment records in Point Click EMR.
• Adjust care plans as needed to ensure optimal outcomes.

Qualifications

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited institution.
• Active Physical Therapy license in California (required).
• Minimum 1 year of experience, preferably in a SNF or rehabilitation setting.
• Excellent communication, interpersonal, and teamwork skills.
• Compassionate and patient-centered approach to care.
