Job Description:
We are seeking a skilled Physical Therapist to provide high-quality rehabilitation services in a skilled nursing facility setting. The role may involve floating between two nearby facilities to ensure patient care continuity. The ideal candidate is passionate about helping patients regain mobility, strength, and independence through individualized therapy plans, with prior SNF experience preferred.
Key Responsibilities:
- Assess, plan, and implement physical therapy treatment programs tailored to each patient’s needs.
- Monitor and document patient progress, adjusting therapy plans as necessary.
- Collaborate with interdisciplinary teams to optimize patient care and outcomes.
- Provide patient and family education on exercises, mobility techniques, and safe practices.
- Ensure compliance with facility policies, procedures, and documentation standards.
Qualifications & Skills:
- Active Physical Therapist license in California.
- Minimum 1 year of experience in physical therapy, preferably in a skilled nursing or rehabilitation setting.
- BLS/BCLS certification required.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a collaborative team.
- Flexibility to float between facilities as needed.
