Physical Therapist (PT) – Days

Location: Adena Greenfield Medical Center – Greenfield, OH
Duration: 13 Weeks (Possible Extension)
Schedule: Day Shift | Monday–Thursday 7:00 AM–6:00 PM (negotiable) & Friday 7:00 AM–3:30 PM

Position Overview:

We are seeking a dedicated and motivated Physical Therapist (PT) to join our rehabilitation team at Adena Greenfield Medical Center. The ideal candidate will provide high-quality, evidence-based therapy to patients across a variety of conditions and diagnoses.

This role requires excellent clinical judgment, strong interpersonal skills, and a commitment to improving patients’ functional outcomes through compassionate, individualized care. The therapist will also serve as a mentor and supervisor to support staff and therapy assistants involved in patient treatment.

Key Responsibilities:

• Evaluate, plan, and implement individualized treatment programs for patients based on clinical findings and physician recommendations.
• Deliver high-quality physical therapy interventions to improve mobility, strength, balance, and overall function.
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
• Maintain accurate and timely documentation within the electronic medical record (EMR).
• Educate patients and families on treatment goals, exercises, and home programs.
• Collaborate with a multidisciplinary care team to ensure comprehensive patient care.
• Supervise and provide feedback to therapy assistants involved in patient treatment.
• Contribute to departmental initiatives focused on quality improvement and best practices.

Qualifications & Requirements:

• Active Ohio Physical Therapist License – Required
• BLS Certification (AHA) – Required
• Graduate of an accredited Physical Therapy program – Required
• 1–2 years of recent physical therapy experience – Preferred
• Strong documentation, clinical reasoning, and communication skills
• Proven ability to manage time effectively and work collaboratively in a team environment
