Job Overview:
We are seeking a skilled LPN with experience in long-term care to provide high-quality, patient-centered care in a LTC setting. This 13-week contract involves performing assessments, treatments, medication administration, ADL assistance, gastrostomy tube care, urinary catheter care, nebulized treatments, wound vac care, CPAP care, and other assigned duties. The LPN will communicate pertinent information to MDs/NPs and assist with patient admissions and discharges.
Key Responsibilities:
- Deliver safe and effective patient care across various LTC procedures.
- Perform assessments, administer medications, and provide specialized treatments as required.
- Assist patients with activities of daily living and ensure comfort and safety.
- Document care accurately in Meditech EMR.
- Notify providers of pertinent changes in patient condition and participate in care planning.
Requirements:
- Active LPN license.
- Minimum 2 years of LTC experience.
- BLS/BCLS certification required.
- Proof of flu vaccination or approved medical/religious exemption.
Additional Information:
- Variable shifts based on census and facility needs.
- Holiday coverage may be required.
- Orientation and employee health clearance required prior to start.
