RN-LTC/SNF

• 🕒 Shift: 12-hour days, 6:00 AM – 6:00 PM
  👩‍⚕️ Position: RN (No LPN)
• 🏥 Facility: St. Joseph’s Villa and Court, David City, NE
• Non-profit licensed facilities
• Skilled nursing (St. Joseph’s Villa)
• Assisted living (The Court)

• 📋 Submittal Qualifiers:
• ✅ No current/prior discipline on license or certification
• ✅ Minimum 2 years RN experience in LTC
• ✅ No criminal convictions other than minor traffic violations
• ✅ Active RN license (Nebraska or compact state)
• ✅ Active BLS (AHA)

• 📄 Requirements:
• Resume with education (month/year) & complete work history (last 7 years, facility names)
• Skills Checklist (completed within 1 year)
• Two references (1 supervisor minimum)
• Online license verification (Nursys) within 30 days
• Drug screen (10-panel) within 30 days
• Physical exam within 1 year
• Vaccinations: MMR, Varicella, Hep B, Influenza, Tdap, TB documentation
• Background checks: National Criminal, FACIS III, Sex Offender, OIG, SAM
• Specialty Competency Exam (80% passing, annually)
• Joint Commission Modules Core I, II, III
• Valid state-issued ID or driver’s license
• COVID-19 vaccination or declination
• Nebraska DHHS Registry Check
• Smoke-free campus & pager policy acknowledgment

• 🩺 Department Notes:
• Two hallways: St. Maria & St. Frances (LTC residents)
• St. Theresa’s Garden: Memory Support (avg 8 residents)
• Mix of single/double rooms
• Charting: American Health Tech (like PCC)
• Orientation: Wednesdays, 10:00 AM – 3:00 PM

• 📊 Staffing Ratios:
• Day: 1 RN/LPN per hallway + St. Theresa’s Garden, 1 CNA per hallway & bath aide, 1 CNA in Memory Support
• Night: 1 RN/LPN for entire facility, Med Aide 6:00 PM–10:00 PM, 2 CNAs, Med Aide for Memory Support

• 💊 Medications:
• Cassettes changed every Thursday night
• Night shift travelers assist with med changeover
• Occasional IV fluids
• ⏱️ Guaranteed Hours: 36
