LPN - LTC/Assisted Rehab

🕕 Shift: 12H Nights | 6 PM - 6:30 AM; every other and holiday rotation
📝 Unit Notes / Important Info:
- 80-mile LOCAL Placements
- LOCAL ADJUSTED BILL RATE
- LOCAL ADJUSTED OT BILL RATE
- No Charge Differential at Good Samaritan Facilities
- No on call or call back available
- $500 fee if clinician backs out or quits with less than 14-day notice
- If GSS housing is used, local rates apply
⏱️ Guarantee (Clinical): 36 hours/week for 12-hour shifts, or 40 hours/week for 8–10 hour shifts
📅 Work Week: Sunday through Saturday
⚠️ Cancellation Policy: 2 weeks' notice
💻 Charting System: Point Click Care
🎓 BSN Required: No
📄 License to Submit: Yes
🚨 Trauma Level / Care Setting: Post Acute
