👩‍⚕️ Job Summary / Purpose

Perform & document assessments (appropriate to LPN licensure)
Complete health history & admission process
Make interventions or referrals as needed
Identify discharge planning & education needs
Provide personal patient care
Communicate patient progress & adjust own workload

🔑 Essential Key Responsibilities

Assist RNs or other licensed practitioners in identifying:
- Patient needs
- Care priorities
- Care goals (considering age, ethnicity, economics, lifestyle, beliefs)
Gather, observe, record, & communicate responses to interventions
Collaborate to modify care plans as needed
Show department-specific competency (according to policy/procedure)
Perform other duties as assigned
Be part of a multidisciplinary team
Delegate nursing tasks only to competent personnel
Spot early warning signs in patient condition & report ASAP
Carry out nursing interventions & medical regimens safely & promptly
Accurately document all care provided
Share timely, relevant info with patients, families & the team
Report patient pain symptoms promptly
Participate in health education (approved by licensed practitioner)
Help with discharge planning, education & counseling

Guaranteed Hours: 36/week
