Job Summary

The Physical Therapist is responsible for evaluating physical therapy needs and delivering comprehensive, medically prescribed treatment to patients in a supervised rehabilitation setting. The role includes developing individualized treatment plans based on patient capacity and setting realistic, achievable goals.

Responsibilities

• Document and record patient conditions and provide education to patients and families regarding physical therapy plans.
• Perform initial assessments and evaluations for designated patient populations.
• Review medical charts to obtain relevant clinical information and clarify referrals when needed.
• Select appropriate assessment tools and techniques to accurately determine patient needs and create optimal treatment plans.
• Request treatment orders when needed for unfamiliar diagnoses.
• Develop and implement discharge plans in collaboration with patients, caregivers, and care team members.

Qualifications

• Bachelor’s Degree in Physical Therapy
• Active New York State Physical Therapy License
• Strong communication and interpersonal skills
• 0–1 year of experience as a Physical Therapist
• Bilingual (Spanish) strongly preferred

Compensation

Salary: $95,000–$100,000
