Description

Title- LTC- LPN/LVN 
Facility name- Sullivan County Health Care
Location- 5 Nursing Home Dr, Claremont, NH 03743
Shift-  Varied Shifts
Contract: 13 weeks 
Pay Term- Weekly (By Every Friday)
Benefits: Paid orientation & sick leave (13)

Gross Weekly- $2,014/week
Guaranteed Hours- 48hrs/week
 
Breakdown-
W2-$16/hr
Stipend- $1,246/week

Bonus-

Travel bonus - $500

Contract Completion bonus- $500

Job Description:

Shift Notes:
Weekends days (7-3) and evenings (3-11) and varied during the week dependent on need/Must be willing to float to units where experienced/48 hours guaranteed

Submission Requirements (Must be in profile at upload)

3+ years current LPN/LVN experience (within last 6 months) – Required
Willing to float – Required
IV certified (or willing to obtain during onboarding) – Required
COVID vaccination – No exemptions accepted
RTO: Must be listed at submission; max 5 days; holidays not approved
Locals NOT accepted (must live 50+ miles from facility)
Previous travel experience – Highly preferred
Rural/small facility experience – Highly preferred

Certification Requirements

Active certifications (BLS/AHA, IV if applicable)
Active NH or Compact License – must be in hand at submission
COVID vaccine documentation
