Job Summary:
We are seeking an experienced and compassionate Physical Therapist Assistant (PTA) to join a skilled nursing facility, providing high-quality rehabilitative care to patients in a short-term rehab and long-term care setting. The ideal candidate will have prior SNF experience and be comfortable supporting therapy services across two nearby facilities, as needed.
Responsibilities:
- Provide therapeutic interventions under the direction and supervision of a licensed Physical Therapist.
- Assist patients with strengthening, mobility training, gait training, and functional exercises.
- Support patient progress toward individualized treatment goals and document daily sessions per facility standards.
- Communicate effectively with the supervising PT, interdisciplinary team, and facility staff regarding patient progress and needs.
- Maintain accurate and timely documentation within the facility’s EMR system.
- Follow established care plans and ensure safe, appropriate use of therapy equipment.
- Float between assigned facilities (within walking distance) when required.
Requirements:
- Active Physical Therapist Assistant (PTA) license in the state of California.
- Minimum of 2 years of experience, preferably in a Skilled Nursing Facility (SNF).
- Current BLS certification.
- Strong communication, time management, and patient-centered care skills.
- Ability to work both independently and collaboratively in a multidisciplinary environment.
Assignment Details:
- Full-time schedule, 40 hours per week.
- Monday–Friday preferred; Tuesday–Saturday schedule also acceptable.
- No holiday coverage required.
Setting:
- Skilled Nursing Facility (SNF) environment.
