Job Description

This role is best suited for a Physical Therapist with strong acute inpatient experience. The primary responsibility is providing comprehensive physical therapy services in an acute rehab environment, with secondary coverage in ICF/SNF rehabilitation settings.

The PT will evaluate and re-evaluate patients, develop and implement individualized treatment plans as prescribed by the attending physician or consulting specialist, and deliver high-quality therapeutic interventions. The role requires flexibility to work various shifts, weekends, and holidays depending on operational needs.

Key Responsibilities

  • Conduct initial and ongoing evaluations for assigned patients.
  • Develop individualized therapy plans focused on functional rehabilitation.
  • Deliver skilled physical therapy interventions according to physician orders.
  • Provide consultative PT services as needed.
  • Document all patient interactions thoroughly within the EMR system.
  • Collaborate with multidisciplinary teams to support patient recovery.
  • Support rehab programming in acute, acute rehab, and ICF/SNF settings.
  • Maintain compliance with all facility, state, and professional regulations.
  • Participate in weekend and holiday coverage as required.

Requirements

  • Active Hawaii PT License by assignment start (must apply upon acceptance).
  • BLS certification (valid and unexpired).
  • Minimum 6 months of experience working with adult patients in at least one of the following: acute care, rehab hospital, or skilled nursing facility.
  • Ability to work variable schedules, including weekends and holidays.
  • Experience documenting in Meditech or comparable EMR is preferred.

Additional Notes

  • Candidates must arrive the Thursday before the assignment begins to complete employee health requirements on Friday (mandatory).
  • Compliance items must be completed before the assignment start date.
