Position: LPN
Setting: Post-Acute
Division: Nursing
Contract Length: 13 weeks (91 days)
Guaranteed Hours: 36 hrs/week

Shift: 12-hour Nights (may occasionally work Days)

Scheduling Requirements

Weekends: Minimum every 3rd (more if needed)

Holidays: Minimum every 3rd (more if needed)

Shifts: 12-hour shifts (no 8s at this location)

IMPORTANT>>>>>>

**Must have at least 1yr of recent experience in LTC setting.**

Candidates with below the minimum experience required and incomplete requirements will not be entertained

>>Current Resume - MUST include Facility Name, Month & year for duration on each facility, charting used (EPIC, PCC etc) and Duties for each facility listed on resume

>> License and BLS must not be expiring in the next 6mos, otherwise provide proof/receipt of renewal attempt
 

>> 1 supervisor reference within the last year worked in the unit
being submitted (indicate name, facility and year)

>> 1 peer reference within the last year worked - please
ensure dates of employment are on these (indicate name, facility and year)

Special Notes / Policies

• Local Radius Rule: Local rates apply within 80 miles or if using GSS housing
• Backouts/Quits: Less than 14-day notice = $500 fee
