HYBRID Remote Primary Care FNP - Bilingual Spanish (Downtown Manhattan - must live in NYC
Nurse Practitioner MUST be bilingual / Fluent in SPANISH language

We are hiring several Board-Certified Family Nurse Practitioners with active NY State Provider license in good standing who come with at least 1 year Provider experience in either Family Medicine, Internal Medicine, or Primary Care.
• Provider will provide direct patient care to all age ranges and will improve quality outcomes and patient satisfaction. The Provider will report to the Medical Director, and will collaboratively provide outstanding care to a multicultural patient population.
• Scope of Responsibilities:
• Provider collaborates with Specialists that share the primary care clinic or work in nearby neighborhoods
• Provide a wide range of comprehensive primary care services to patients of all ages, performing physical exams, diagnosing and treating illnesses, prescribing medications, managing chronic conditions, and educating patients about healthy lifestyles.
• Order and interpret diagnostic tests, develop treatment plans, and refer patients to specialists when needed.
• Develop and implement treatment plans, prescribe medications, and manage chronic conditions like diabetes and hypertension.
• Educate patients on preventative care, healthy lifestyle choices, and disease management.
• Maintain accurate and up-to-date patient records, documenting medical history, assessments, diagnoses, and treatment plans.
• Administer immunizations to prevent infectious diseases.
• Participates in quality improvement efforts designed to improve clinical performance and maintain compliance
• A commitment to clinical interventions and delivery models that achieve the goals of population health and financial sustainability
• Contributes to the development of policies and standards of patient care by participating in committees.
• Complies with company policies & procedures related to privacy and security of identifiable patient information (PHI), as explained in the HIPAA (Health Insurance Portability & Accountability Act) training.
• Supports the stated goals and objectives of the Patient Centered Medical Home by working cooperatively with all clinical and non-clinical staff to develop procedures that enhance the Health Center’s team-based approach to patient care
