List all facilities the traveler is willing to work at (ex: Any AZ, Any CO, Any state). List all acceptable shifts and check all departments the traveler is willing to work. Include all experience areas the traveler will accept — required on cover page. Candidates must be pre-closed and ready to accept if offered. Notify immediately if a candidate withdraws or becomes unavailable. Important This form must be 100% accurate with all requests noted in the highlights section.
