🏥 Facility: Health Care Connectors – CT
🕒 Shift: 5x8 Days, 8:30 AM to 5:00 PM, alternating weekends
📅 Start: 12/15/25 or sooner
⏳ Length: 13 weeks
💼 Guaranteed Hours: 40
💰 Weekly Pay: $2,628

Position Details:
This is a Home Health RN role covering the Greenwich area. Candidates must have at least 2 years of Home Health experience and be fully licensed at the time of submission. Strong documentation skills are required.

Requirements:

2+ years Home Health RN experience
Experience with OASIS and Homecare Homebase
Experience with WellSky and wound care preferred
On-call/tr iage experience
Reliable, organized, and professional
Strong time management and customer service skills
Compassionate, patient-focused approach

On-Call Expectation:

One to two weeks per month
On-call hours: 5:30 PM to 8:30 AM, weekdays
