Dress Code

• ACLS: Advanced Cardiac Life Support
• BLS: Basic Life Support
• PALS: Pediatric Advanced Life Support
• NRP: Neonatal Resuscitation Procedure
• CHEMO: Chemotherapy
• EFM: Electronic Fetal Monitoring
• NURSYS Verification: Required for all RNs
• IV Certification: Required for LVN/LPN/Other
• Management of Assaultive Behavior (MAB): Required
• NIH Stroke Certification: Required
• CPI: Crisis Prevention Institute
• Medication Test/Pharmacology Competency Exam: Required as applicable by profession

Health Documents Required

• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO.
• TB Test: IGRA/TSPOT, QGold, or 2 PPDs within 12 months of start date.
• Vaccinations:

  • Tdap Vaccine
  • Flu Vaccine (during flu season)
  • COVID-19 Vaccination or Declination
  • Hepatitis B Vaccination
  • Varicella Vaccination
• Drug Testing: 10 Panel Drug Screen within 90 days of start.

Background Check

• Required within 45 days of start covering all locations lived/worked within the past 7 years.

References

• Minimum of 2 references covering 1 year of employment total within the last three years within a comparable specialty/setting.
