All facilities the traveler is open to working in must be listed on the submission. If the traveler is open to any location, please specify (e.g., Any AZ, Any CO, Any State, etc.) All shifts the traveler is willing to work must be included on the submission. All departments the traveler is willing to accept must be checked/selected on the Banner cover page. All candidates submitted must be pre-closed and ready to accept an offer. If the traveler withdraws or becomes unavailable, please notify immediately.
