Job Description

• Proficient in at least one department: Chemistry, Hematology, or Blood Bank
• ASCP certification required

Licensure & Certification Requirements

• ASCP certification mandatory
• Primary source verification of professional license/certification required within 30 days of start date
• MA Conditional Licenses not accepted

Health & Immunization Requirements

• MMR: Documentation of two measles, two mumps, and one rubella vaccine OR two MMR vaccines, or proof of immunity via blood test
• Influenza vaccine required during flu season (Sept 15 - April 30); no exemptions allowed
• TB test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date; updated if >6 month break
• Health statement signed by MD/DO/NP/PA within 365 days of start date; updated if >6 month break
• Hepatitis B vaccine or immunity proof strongly preferred
• Tdap vaccine within 10 years preferred
• Positive PPD requires negative chest x-ray documentation (optional but recommended)

Professional Requirements

• Resume including past 7 years of work experience with city & state required for submission
• Two recent references required (minimum one supervisor or above in skillset)
• Skills checklist required for submission
• Criminal background check within 45 days prior to start date, updated if >6 month break
• Massachusetts CORI background check within 30 days prior to start date, updated if >6 month break
• Education verification of highest level achieved required
• Social Security Number trace for past 7 years required
• National sex offender registry check within 30 days of start, updated annually
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually
• Confirmation of assignment form required upon acceptance and for extensions

Additional Requirements

• MGB NWH Self-Study Orientation & Attestation required before onboarding
• Respirator Medical Clearance Attestation required if fit testing is needed for the role
• Candidate submission coversheet required for submission
