Job Summary

The Nurse Navigator – Inpatient GI is a Registered Nurse with specialized clinical knowledge who provides individualized support to patients, families, and caregivers throughout the gastrointestinal care continuum. This role focuses on improving access, reducing barriers, coordinating services, and ensuring timely, high-quality care.

The Nurse Navigator interfaces with patients both in person and through telephonic/electronic communication. Although patient-facing, this role is not a bedside RN position. It supports the patient journey by providing education, coordinating multidisciplinary care, and advocating for optimal clinical outcomes.

Core Responsibilities

Patient Coordination & Navigation

  • Serve as a subject matter expert in navigation for GI patients.
  • Improve timely access to care and help patients overcome barriers.
  • Collaborate with leaders to analyze community needs and recommend program improvements.
  • Provide relationship-based, culturally sensitive, patient-centered care.
  • Partner with the navigation team to ensure continuity and cross-coverage.

Clinical Assessment & Care Coordination

  • Utilize the nursing process (assessment, diagnosis, planning, implementation, evaluation) to guide patient care.
  • Coordinate with interdisciplinary teams—case management, inpatient units, specialty clinics, and multidisciplinary rounds.
  • Support efficient care pathways, documentation, communication, and patient follow-up.
  • Identify service gaps and help locate needed resources for patients and families.

Professional Practice & Leadership

  • Complete all required competency assessments and ongoing training.
  • Serve as preceptor/mentor to nurses, staff, and students.
  • Stay current with GI, navigation, and oncology-related advancements.
  • Promote evidence-based practice, research utilization, and performance improvement.
  • Uphold organizational policies, regulatory standards, and patient safety goals.

Resource Utilization

  • Use resources efficiently and responsibly in support of care coordination.
  • Step into additional support roles when clinically appropriate and approved.
  • Apply critical thinking and evidence-based solutions to improve patient outcomes.

Required Qualifications

Education

  • Associate Degree in Nursing or Nursing Diploma — Required
  • Bachelor of Science in Nursing (BSN) — Preferred

Licensure & Certifications

  • Active RN License (Single-State or Multi-State Compact) — Required
  • Basic Life Support (BLS) — Required
  • Advanced Cardiac Life Support (ACLS) — Required within 180 days of hire

Experience

  • Minimum 3 years of nursing practice — Required
  • At least 2 years of disease-specific experience (GI, oncology, or related specialty) — Preferred
  • Outpatient or care coordination experience — Preferred
