Physical Therapist (PT) – Skilled Nursing Facility | Natick, MA

About the Role:
MedAdventures is seeking a PT for our skilled nursing facility in Natick, MA. You’ll provide hands-on therapy to residents, helping them improve strength, mobility, and independence while contributing to a coordinated care plan with the rehab team.

Responsibilities:

Evaluate residents and develop individualized treatment plans
Deliver skilled PT services to improve function, mobility, and balance
Maintain accurate and timely documentation
Collaborate with nursing, OT, and SLP teams
Educate residents, caregivers, and staff on safe movement and rehab goals

Requirements:

Active Massachusetts PT license (or eligibility)
Experience in SNF or rehab preferred
Strong communication, organization, and documentation skills
Patient-focused, compassionate approach
