GLC is hiring: LPN LTC/SNF - Middlebury, VT - 16-week contract

About this Assignment
Join the care team in LTC/SNF where you’ll provide patient-centered care in a collaborative environment. Typical responsibilities include direct patient care, timely documentation, and coordination with the care team. Specific duties will be confirmed during your interview with a recruiter.
Assignment Details
- Location: Middlebury, VT
- Assignment Length: 13-16 weeks | Days or nights
- Start Date: 12/22/2025
- End Date: 04/13/2026
- Pay Range: 1808 for 36 hours
Minimum Requirements
- Optional: Active license in LTC/SNF
- 1 year full-time LPN, LTC/SNF experience within the last 2 years
- minimum of 1 year of experience Prior travel required EPIC experience from within the past 2 years is required and nonnegotiable Additional Information: IVS to OFFER Monday to Sunday No block schedules for any reason Bi-weekly orientations on Monday next available is 1/5 Please ensure all employment gaps over 90 days are explained This facility requires ALL work history information (bed count EMR facility type etc.) to be complete in order to submit Disclaimers: Public Transportation is NOT accessible in this area please ensure you have reliable transportation personal vehicle or rental vehicle are strongly recommended Vehicles should be equipped with all season or snow tires during the months of November April
