Key Responsibilities

Clinical & Psychosocial Assessment

  • Conduct timely psychosocial assessments for ED and medical inpatients.
  • Identify social, emotional, financial, environmental, and support system factors impacting health outcomes.
  • Assess for risk factors including abuse/neglect, mental health crises, substance use concerns, capacity issues, and unsafe home environments.

Discharge Planning & Care Coordination

  • Develop, implement, and document comprehensive discharge plans in collaboration with the interdisciplinary team.
  • Coordinate services such as SNF placement, home health, DME, hospice, rehab, community mental health, and transportation.
  • Communicate effectively with physicians, nursing, case management, therapy staff, and outside agencies to ensure safe transitions of care.
  • Facilitate family meetings, care conferences, and complex discharge discussions.

Crisis Intervention & Support

  • Provide immediate social work interventions in the ED including crisis stabilization, safety planning, and linkage to emergency resources.
  • Support patients and families coping with acute illness, trauma, and end-of-life decisions.
  • Assist with advance directives, guardianship issues, and reporting to Child/Adult Protective Services when required.

Compliance & Documentation

  • Maintain complete, timely, and compliant documentation within the EMR.
  • Uphold hospital policies, state and federal regulations, and ethical standards of professional social work practice.

Team Collaboration & Professional Practice

  • Serve as a consultative resource to the care team on psychosocial issues, community resources, and discharge barriers.
  • Participate in interdisciplinary rounds, huddles, and quality improvement initiatives as assigned.
  • Demonstrate flexibility in covering ED and inpatient units per departmental needs.

Qualifications

Required:

  • Master’s degree in Social Work (MSW).
  • Active Maryland LMSW or LCSW-C license in hand at time of hire.
  • Minimum 2 years hospital inpatient experience in Med-Surg, critical care, or ED settings.
  • Proven discharge planning and care coordination expertise.
  • Ability to work 10-hour shifts and required weekend/holiday rotations.
  • Strong crisis intervention skills; comfortable working in fast-paced acute care environments.

Preferred:

  • Experience in both ED and medical inpatient units.
  • Familiarity with community resources, behavioral health systems, and acute-care workflows.
  • Excellent communication, problem-solving, and conflict-resolution skills.
