Description

Physical Therapist (PT) – Home Health

Location: Tallahassee, FL & Surrounding Areas
Type: 13-Week Contract
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (40 hrs/week)
Productivity: 30 Units
On-Call: Required only if requested
Travel Area: Tallahassee, Crawfordville & Gadsden County

Position Overview

We are seeking a skilled Home Health Physical Therapist to provide patient-centered rehabilitation care in the home setting. The PT will manage a diverse caseload while maintaining productivity, quality documentation, and positive clinical outcomes.

Qualifications

License: Active Physical Therapist license in the state of Florida – Required
Experience:
- 1–2 years as a PT in acute care or rehab setting – Required
- Previous home health experience – Preferred
Software:
- Homecare Homebase (HCHB) – Preferred
- OASIS familiarity – Preferred/Required based on agency standards
Certifications & Requirements:
- Current CPR certification – Required
- Valid driver’s license, reliable vehicle & auto insurance – Required

Education

Bachelor of Science in Physical Therapy from an APTA-accredited program – Required

Must have previous experience Homecare Homebase (HCHB) and OASIS Locals accepted with the pay rate of $2560/wk
