• Day shift: Monday–Friday, 8:00 AM – 4:30 PM
• No on-call required
• Average daily census: 26
• Community-based in patients’ homes (office located on campus)
• Reliable vehicle required

Job Title

• Physical Therapist – Home Health

Start Date & Schedule

• Start Date: ASAP (approximately 4 weeks from offer to complete compliance)
• Shift: 5x8, Monday–Friday, 8:00 AM – 4:30 PM (start time can be flexible)
• Guaranteed Hours: 40 per week
• Weekend Required: No
• On-Call Required: No
• Block Schedule: Accommodated (M–F)

Experience Requirements

• 1 year Home Health / OASIS experience required
• 1 year EPIC experience preferred
• First-time travelers accepted if OASIS experience is present

Licensing & Certifications

• Alaska PT License required (Compact PT allowed)
• BLS required
• Pending license accepted: No

Floating Information

• May float to Hospice if needed
• No floating to sister facilities

• COPD
• CHF
• Orthopedic procedures (including joint replacements)

Job Summary

• Evaluate patients and determine plan of care and treatment goals
• Provide and oversee physical therapy treatments within scope of practice
• Supervise therapy assistants, students, aides, and/or volunteers as applicable
• Collaborate with the interdisciplinary care team to optimize patient outcomes

Essential Functions

• Evaluate patients using records, interviews, observation, and collaboration
• Perform and modify treatment plans as needed
• Complete documentation accurately and on time per regulations
• Communicate with healthcare team to support continuity of care
• Mentor and supervise assistants, aides, and students
• Participate in program development, staff meetings, and quality improvement
• Perform other assigned duties as needed

Education Requirements

• Bachelor’s Degree in Physical Therapy (required) or
• Master’s Degree in Physical Therapy or
• Doctorate in Physical Therapy
• Must be from a program accredited by the Commission on Accreditation in Physical Therapy Education (foreign equivalent accepted)
