Job Description

Required Credentials & Onboarding Requirements
Licenses & Certifications
- State Nursing License: Verification required within 30 days prior to start
- BLS (American Heart Association): Required before onboarding
- ACLS (American Heart Association): Required before onboarding
Professional Requirements
- Arnot Skills Checklist: Must be fully completed and updated annually for the unit assigned
- Arnot Core Competency Exam: Must score 80% or higher on Core Comp I, II (Nursing), and III for RNs
- Driver’s License or State ID: Valid, current, and legible US driver’s license or ID required
- Arnot Resume: Must include all employment and gaps for the last 7 years, education history with degree titles and graduation dates, and locations of assignments
- Professional References: Two required (1 supervisor reference from last 12 months, 1 supervisor or peer reference from last 3 years)
- Employment Verification: Verification of most recent employer required
- Background Checks: Agency background check within 30 days of start, including national and county criminal searches, FACIS 3 check, and 7 instant background checks (manual upload required)
- Drug Screen: Must be completed within 30 days of start; testing excludes marijuana per New York state regulations
- Color Headshot: Recent, professional photo required (no filters, sunglasses allowed only if prescription)
- Arnot Job Description: Signed and dated acknowledgment required
- Arnot Health Personal Information Form, Code of Conduct, Sexual Harassment Acknowledgement, Confirmation of Assignment Form, Self Study Packet Documentation and Quiz, Parking Registration Form: All required and found in Document Library
Personal Health History & Immunizations
- COVID-19 Vaccine: Optional; proof of vaccine or agency declination accepted
- MMR: Positive IgG titer or 2 vaccine series required; must show results for each component
- Varicella: Positive IgG titer or 2 vaccine series required; documentation must include dates and results
- Hepatitis B: Positive antibody titer, 3 vaccine series, or facility-specific declination form required
- Influenza Vaccine: Required or facility-specific declination form; if declined, mask must be worn at all times on assignment
- TB Test: Annual requirement; must have one of the following within required timeframes:
- Two PPD skin tests (one within 12 months, one within 3 months) or
- One Quantiferon/Spot test within 12 months
- Past positives require chest x-ray within 12 months
- Fit Test: Annual requirement; must be completed prior to start using approved mask models
- Tdap: Preferred but not required; if no record, facility-specific declination form required
- Physical/Health Statement: Must be from within last 12 months, include physician credentials, vitals, height, weight, Snellen eye chart, and statement of clearance with no restrictions
Additional Notes
- All documentation must be current and complete prior to onboarding.
- Annual requirements include physical, TB testing, fit testing, and influenza vaccine or declination.
- Background checks and drug screens must be completed within 30 days of assignment start.
- Facility-specific forms and acknowledgments must be completed as part of onboarding.
