LPN - LONG TERM CARE

Shift: 12H Nights, every 3rd weekend
Description / Unit Notes: Good Samaritan
Items of Note:
- Local rates apply within 80 miles o
- No Charge Differential at Good Samaritan Facilities
- No on call or call back available or billable
- Backouts & quits with less than 14-days notice incur a $500 fee
Facility: GSS - Park River
Position: LPN
# Beds: 66
Ratios: 22-25:1 (currently experiencing low census)
EMR: PCC
Weekend Requirement: Every other weekend
Holidays: Every other holiday
Shifts (8s or 12s): 8 hr.; currently have AMs and NOCs working 12s
Location specific orientation: Follow another nurse for 1 shift
Common diagnosis / Types of patients: CHF, dementia, DM, CKD, COPD, dysphagia (PEG tube), urinary retention (foley catheters), wounds
Notes entered: 4.18.25
Cancellation Policy: Client can give 2 week's notice
