Description

*Requirements:
*MS or Compact state license required
*Day shifts/ 8a-4:30p with rotating weekend shifts/ 8 hours shift/5 shifts per week
*Locals are not allowed/travelers must be greater than 50 miles away from the facility
*Must have BLS, must have experience in both discharge planning and UR, 3+ years of Acute Hospital Case Management experience (preference for exp in travel)
*Additional facility info: Cerner is our EHR, CM uses MorCare for documentation and CarePort for referral system. We are transitioning to a new referral system Oct. 28.
