Position Overview

We are seeking a dedicated and compassionate Physical Therapist to join a skilled nursing rehabilitation team. This role involves providing patient-centered care to help individuals regain independence, improve mobility, and enhance their quality of life in a post-acute environment.

Responsibilities

Perform evaluations and develop individualized treatment plans
Implement therapy programs to support functional improvement
Document patient progress and adjust plans as needed
Collaborate with interdisciplinary team members to achieve patient goals
Educate patients, family, and caregivers on therapy techniques and continuing care

Requirements

Active state Physical Therapist license
Less than 1 year of experience acceptable
Ability to work variable shifts
No bilingual requirement
Holiday coverage not required
