Duration: 13 Weeks
Guaranteed Hours: 36 hours/week
Shift Length: 12 hours

Job Summary:
The Licensed Practical Nurse (LPN) provides direct care to patients ready for discharge but experiencing delays. Working under RN supervision, the LPN collaborates with a multidisciplinary team and supervises LPNs, CNAs, and UAPs, applying nursing principles within the scope of practice.

Roles & Responsibilities

• Perform focused assessments of patients’ priority conditions under RN guidance.
• Implement and evaluate focused plans of care as directed by the RN.
• Monitor and document patient responses and progress toward expected outcomes.
• Assist with patient mobility, hygiene, medication administration, and other daily care needs.
• Assign appropriate care tasks to CNAs, UAPs, or other LPNs based on patient needs and staff skill levels.
• Supervise and support team members in carrying out assigned tasks according to RN plan of care.
• Accurately document patient assessments, interventions, and outcomes in the electronic health record (EpicCare Inpatient).

Required Skills

• Strong clinical knowledge of practical nursing, acute care, and discharge planning
• Proficiency in patient assessment, documentation, and nursing interventions
• Ability to collaborate effectively within a multidisciplinary healthcare team
• Excellent communication, interpersonal, and organizational skills
• Critical thinking and problem-solving abilities in dynamic healthcare settings
• Ability to supervise, mentor, and assign tasks to support staff safely and effectively
• Flexibility, adaptability, and resilience in high-stress situations

Qualifications

• Current Oregon State LPN license
• Minimum 2 years of acute care experience
• Current BLS certification
• Experience with electronic health records (EpicCare Inpatient preferred)
