Description
Job Title: Licensed Practical Nurse (LPN/LVN) – Discharge Unit (RDU)
Location: Klamath Falls, OR 97601
Unit: Med/Surg
Shift: Nights - 3*12 hrs
Duration: 13 Weeks
Guaranteed Hours: 36 hours/week
Shift Length: 12 hours
Job Summary:
The Licensed Practical Nurse (LPN) provides direct care to patients ready for discharge but experiencing delays. Working under RN supervision, the LPN collaborates with a multidisciplinary team and supervises LPNs, CNAs, and UAPs, applying nursing principles within the scope of practice.
Roles & Responsibilities
- Perform focused assessments of patients’ priority conditions under RN guidance.
- Implement and evaluate focused plans of care as directed by the RN.
- Monitor and document patient responses and progress toward expected outcomes.
- Assist with patient mobility, hygiene, medication administration, and other daily care needs.
- Assign appropriate care tasks to CNAs, UAPs, or other LPNs based on patient needs and staff skill levels.
- Supervise and support team members in carrying out assigned tasks according to RN plan of care.
- Accurately document patient assessments, interventions, and outcomes in the electronic health record (EpicCare Inpatient).
Required Skills
- Strong clinical knowledge of practical nursing, acute care, and discharge planning
- Proficiency in patient assessment, documentation, and nursing interventions
- Ability to collaborate effectively within a multidisciplinary healthcare team
- Excellent communication, interpersonal, and organizational skills
- Critical thinking and problem-solving abilities in dynamic healthcare settings
- Ability to supervise, mentor, and assign tasks to support staff safely and effectively
- Flexibility, adaptability, and resilience in high-stress situations
Qualifications
- Current Oregon State LPN license
- Minimum 2 years of acute care experience
- Current BLS certification
- Experience with electronic health records (EpicCare Inpatient preferred)
