Job Description: Home Health Case Manager

Position Summary

The Home Health Case Manager is responsible for coordinating and managing patient care in the home setting. This role ensures patients receive high-quality, individualized plans of care, facilitates communication among interdisciplinary team members, and supports safe, effective transitions throughout the episode of care.

Key Responsibilities

• Conduct initial and ongoing patient assessments in the home
• Develop, implement, and regularly update individualized plans of care
• Coordinate care with physicians, therapists, social workers, aides, and other team members
• Monitor patient progress and adjust care plans according to clinical needs
• Educate patients and families on disease processes, treatments, and self-management strategies
• Ensure timely and accurate documentation in the EMR
• Participate in case conferences, quality reviews, and interdisciplinary planning
• Ensure compliance with agency policies, state/federal regulations, and Medicare/Medicaid requirements
• Communicate changes in patient condition to the care team and physician
• Support patient discharge planning and transition of care
• May require rotating on-call duties depending on agency needs

Required Qualifications

• Active RN license (or specify alternate license if needed: LPN, PT, MSW, etc.)
• Minimum 2 years of clinical nursing experience
• Strong assessment and critical-thinking skills
• Excellent communication, time-management, and organizational abilities
• Current driver’s license, auto insurance, and reliable transportation
• Ability to work independently in the home setting

Preferred Qualifications

• Prior home health case management experience
• Familiarity with OASIS documentation (if RN role)
• Experience with home health EMRs (HCHB, Kinnser, WellSky, Epic, etc.)
• Experience coordinating care for chronic or complex patient populations
