Facility & Submission Requirements 50-mile radius rule applies. Must include an unexpired driver’s license with submission. Any licensing hits must be disclosed at submission. Unit Details – IMU (Intermediate Care Unit), Cost Center 6010 30 beds – 14 private, 16 semi-private Certs required: BLS, ACLS, NIHSS EMR: Sunrise Staffing & Ratios RN Ratios: Goal: 1:4 Typical: 1:5 Occasionally: 1:6 CNA Staffing: 2–3 CNAs; ratios 1:10–12 Charge Nurse: Takes a full patient assignment on both day & night shift. Clinical Assistant: Works two 12s (with patient assignment) + two 8s (office). Shift, Scheduling & Weekend Requirements 12-hour shifts: Must be on unit by 6:45 AM/PM (shift: 7a–7p or 7p–7a). Self-scheduling allowed with unit approval. Staff may request up to 4 days off/month. Weekend requirements: Fri/Sat/Sun = weekend shifts. Nights: 4 weekend night shifts per 4-week schedule. Days: 4 weekend day shifts per 4-week schedule. True “straight shift” only applies to nights. Day-shifters must pick up minimum 3 night shifts every 4 weeks (not necessarily weekends). Holiday, Floating & Orientation Must be flexible for holiday coverage (perm staff usually cover most). Floating: Required to Tele or Med-Surg; ratios up to 1:6 (rarely 1:7 in Med-Surg). May float to Pandemic Unit. Orientation: Day 1: ~4 hours with HR + 8 hours on unit with preceptor Day 2: Full shift with preceptor Then independent Clinical Expectations Tele: Nihon Kohden RN must read & interpret their own strips (Tele Techs available). Drip experience required: Amiodarone, Cardizem, Dopamine, Dobutamine, Labetalol, Nitro, Heparin, occasional Lasix. Respiratory: All equipment managed by RT (AirVo, Bipap). RT 24/7. IV Support: IV team 24/7 (they handle all IV starts). PICC/Midline team available. Hospitalist: In-house 24/7 Intensivist: In ICU 24/7 Residents: In-house 24/7 Pharmacy: In-house 24/7 (Pyxis) Typical Patient Population Chest pain, r/o MI CHF, arrhythmias Post-pacemaker Pre/post cardiac cath (manage radial bands, remove—do not pull sheaths) Groin checks Intermediate-care overflow Tikosyn therapy Rare chest tubes
