Job Title: Inpatient Case Manager RN
Job Location: Los Angeles CA
Duration: 13 weeks
Shift Timing: 03:00 PM to 03:00 AM
Shift hours: Evenings 3pm-4am 2(12 hour shifts), 3pm-12am 2(8 hour shifts); weekends Holidays
Job Description:
Must have DC, compact state and Oregon RN license before start
Case Manager/Utilization Management (90%), Discharge Planning
National Care Coordination Command Center
Utilization Management - Develop plans/procedures to help improve outcome of reviews
InterQual, MCG
Pilot project to support in Coordination of Care for patients coming out of hospital.
Plans, develops, assesses and evaluates care provided to members. Collaborates with physicians, other members of multi-disciplinary health care team and patient/family in the development. Reviews all inpatient admissions with 24 hours and begins the discharge planning process.
