Location: Tarzana, CA 91356
Contract: 13 Weeks
Shift: Days – 5×8hr (08:00–16:30)

Pay Package:

Weekly Gross: $2,370/week

Travel Stipend: $1,710/week
Base W2 Rate: $16.50/hr

Local Pay: $45.00/hr. on W2

Job Description

Seeking an experienced LVN/LPN Case Manager for acute care Case Management & Utilization Review. Responsible for coordinating safe patient transitions, ensuring medical necessity, reviewing level of care, discharge planning, and payer authorization processes. Works closely with interdisciplinary teams and post-acute services.
