Location- 168 Industrial Dr, Northampton, MA, 01060
Shift- Days – 8:00 AM–4:30 PM
Contract: 13 weeks
Pay Term- Weekly (By Every Friday)
Benefits: Paid orientation & sick leave (13)

Job Description:

Monday–Friday shift, 8:00 AM–4:30 PM with a 1:4 weekend rotation. Under the direction of the Clinical Team Manager and in alignment with organizational policies, the RN Case Manager provides skilled nursing care and full case management to patients and families in the home setting. Responsibilities include:

Delivering nursing care consistent with the Nurse Practice Act and VNA standards
Using nursing theory and processes to assess, plan, implement, and evaluate care
Documenting assessments, interventions, and patient responses
Teaching, counseling, and promoting patient health
Coordinating care needs and collaborating with the interdisciplinary team

Qualifications

Active RN license in the state of Massachusetts.
Current BLS certification.
Graduation from an accredited School of Nursing (BSN preferred).
Valid driver’s license and reliable transportation.
Home health or case management experience preferred but not required.
Strong assessment, critical thinking, and organizational skills.
