Job Summary

We are seeking a dedicated Home Health Care Registered Nurse for a full-time contract assignment. In this role, you will provide patient-centered home health care services, complete OASIS visits, and work autonomously while being part of a collaborative continuum of care. This position offers consistent weekly scheduling with no on-call expectations and a limited weekend rotation.

Key Responsibilities

Deliver high-quality nursing care to patients in their homes.
Conduct and document OASIS assessments accurately and efficiently.
Coordinate care plans and communicate effectively with interdisciplinary teams.
Provide patient and family education to support ongoing recovery and wellness.
Maintain compliance with documentation standards and regulatory requirements.

Required Qualifications

Active RN license (must be current).
Minimum of one year Home Health Care experience required.
BLS certification required.
Ability to independently conduct OASIS visits.
Strong communication and organizational skills.
