Description
Position Summary:
The Home Health Registered Nurse (RN) delivers comprehensive nursing care to patients in their homes, focusing on promoting recovery, independence, and optimal health outcomes. The RN coordinates individualized care plans, performs skilled nursing interventions, and collaborates closely with physicians, caregivers, and interdisciplinary team members to ensure high-quality, patient-centered care.
Key Responsibilities:
- Perform in-home patient assessments, including physical, emotional, and environmental evaluations.
- Develop, implement, and revise individualized patient care plans in accordance with physician orders and clinical guidelines.
- Provide skilled nursing care such as wound care, medication management, IV therapy, catheter care, injections, and chronic disease management.
- Monitor changes in patient condition and communicate timely updates to the physician and care team.
- Educate patients and families on disease processes, medications, home safety, and self-care techniques.
- Coordinate care with interdisciplinary team members including PT, OT, ST, social workers, and home health aides.
- Document all patient encounters accurately and promptly in the electronic medical record system.
- Ensure compliance with home health regulations, agency policies, and quality standards.
- Participate in case conferences, performance improvement activities, and staff meetings.
- Promote patient safety in the home environment and advocate for necessary support services or equipment.
- Maintain a high level of professionalism, compassion, and patient confidentiality.
Qualifications:
- Active and unrestricted RN license in the state of practice.
- Minimum of 2 year of recent clinical nursing experience, preferably in home health, med-surg, or a related field.
- Strong assessment and critical-thinking skills.
- Valid driver’s license, reliable transportation, and auto insurance.
- Current BLS certification (required).
- Experience with electronic charting (e.g., Homecare Homebase, Epic, Cerner) preferred.
- Ability to work independently in a home environment and travel within assigned geographic areas.
- Excellent communication, organizational, and patient-teaching skills.
