Position Overview

Seeking an experienced Occupational Therapist (OT) for a Home Health contract assignment in the Sacramento, CA region. This role requires strong clinical assessment skills, efficiency in documentation, and the ability to manage a productive caseload within a home health model.

Schedule & Expectations

• Hours: Monday–Friday
• Shift: Days
• Weekend Rotation: 1 weekend per month
• Training: 3 full days (1 in-office + 2 in-field)
• Productivity: 30 points/week
• Point System Conversion:
  • 1 point = 1.33 hours
  • Average caseload supports an 8 hr/day, 40 hr/week schedule
• Overtime: Must be pre-approved by Clinical Supervisor

Role Responsibilities

• Prioritize Start of Care (SOC) visits
• Supervise Occupational Therapy Assistants (OTAs)
• Complete mandatory next-day reporting for SOC visits
• Submit all clinical notes/documentation within 48 hours
• Maintain consistent communication with supervisor, intake, and office teams
• Attend mandatory trainings and maintain compliance expectations
• Provide high-quality patient care in a home-based setting

Requirements

• Active CA Occupational Therapist (OT) License
• 1+ year of Home Health experience
• Strong working knowledge of home health OASIS documentation
• Ability to manage productivity requirements and meet deadlines
• Reliable transportation

Mileage & Equipment

• Mileage Reimbursement: $0.655/mile (first to last patient)
• Parking & Tolls: Paid
• Equipment Provided: iPad + Ring Central
• Company Car Option: Available
