Requirements:

License: Active California RN license (non-compact)
Certifications: BLS
Experience:
- 1–2 years of Home Health or Hospice nursing
- Must have Home Care Home Base (HCHB) experience, including Point Care
EMR: Home Care Home Base

Role Overview:
This contract role focuses on providing high-quality hospice nursing care to patients in their homes. The RN will manage patient assessments, care planning, documentation, and coordination with interdisciplinary teams. Most visits include start-of-care assessments, with a point-based visit structure (1 point per standard visit; start-of-care visits count as 2 points).
