Description

Essential Responsibilities

Conduct thorough inpatient rehabilitation evaluations, including functional assessments and mobility screenings.
Develop individualized treatment plans based on clinical findings and patient goals.
Provide therapeutic interventions to restore mobility, reduce pain, and enhance functional independence.
Complete IRF-PAI (Inpatient Rehabilitation Facility–Patient Assessment Instrument) documentation accurately and in compliance with regulatory standards.
Maintain timely and detailed documentation of treatment sessions, progress notes, and patient outcomes.
Collaborate effectively with physicians, nurses, case managers, and interdisciplinary rehabilitation team members.
Educate patients and families regarding rehabilitation goals, home exercises, safety, and discharge planning.
Adhere to hospital, departmental, and regulatory guidelines for patient care and safety.

Required Qualifications

Current California Physical Therapist (PT) License.
Current American Heart Association (AHA) Basic Life Support (BLS) certification.
Doctor of Physical Therapy (DPT) degree or a Bachelor’s in Physical Therapy if graduated 15+ years ago.
Minimum of 1 year of inpatient rehabilitation experience.
IRF-PAI experience required.
