Job Summary

The Physical Therapist provides high-quality rehabilitative care to residents in a Skilled Nursing Facility (SNF) and Long-Term Care setting. This role focuses on improving residents' functional mobility, independence, and quality of life through evidence-based therapy interventions, care planning, and interdisciplinary collaboration.

Responsibilities

• Perform evaluations and therapeutic treatments under physician or provider orders.
• Develop individualized plans of care, including long- and short-term goals and modality selection.
• Collaborate with residents, caregivers, and interdisciplinary team members on care planning and discharge needs.
• Continuously assess resident progress and adjust therapy plans accordingly.
• Document all evaluations, treatments, and resident interactions in the EMR per facility standards.
• Educate residents and caregivers on exercises, mobility, safety, and equipment use.
• Recommend and train residents on appropriate assistive devices.
• Supervise and collaborate with PTAs and other therapy staff as appropriate.
• Maintain compliance with Medicare/Medicaid and other reimbursement regulations.
• Participate in team meetings, in-services, and program development within the Rehab department.
• Support facility infection control procedures and maintain a safe, clean clinical environment.
• Maintain required licenses, competencies, and annual training.

Requirements

• Active Rhode Island PT license (must be active at submission).
• Minimum 3 years of experience required.
• SNF or LTC experience preferred.
• Bachelor’s degree required; Master’s or Doctorate preferred.
• BLS/BCLS certification required or preferred (per facility).
• Strong communication, documentation, and teamwork skills.
• Ability to lift up to 50 lbs (2-person assist for heavier tasks).

• Overtime: Weekly OT after 40 hrs
