Job Summary:
We are seeking an experienced and compassionate Physical Therapist to join our home health team, providing patient-centered rehabilitation services across the San Jose region. The PT will evaluate and treat patients using medically prescribed physical therapy programs designed to restore function, minimize disability, and maximize each patient’s physical potential. Responsibilities include performing initial admissions/evaluations, supervising Physical Therapy Assistants, documenting clinical findings and treatment plans, and communicating OASIS scores within required timelines. The ideal candidate demonstrates strong clinical judgment, excellent documentation skills, and a commitment to delivering high-quality in-home care.

Territory Coverage:
Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale, and surrounding cities as needed.

Requirements:

• Minimum 1 year of Home Health experience
• Active state Physical Therapist license
• BLS/BCLS certification
• Experience working independently in a home health environment
• EMR proficiency (Homecare Homebase HCHB preferred)

Job Details:

• Shift: Days, 8:00 AM–5:30 PM (flexible based on patient availability)
• Weekend Coverage: One weekend day per month
• Holiday Coverage: Required
• Contract Length: 13 weeks
