Travel Pay Rate: (above 50 miles)

• Hourly:$28/hr W2 + $1,246/week (non - taxable stipend)
• Gross:$2,254/weekly

Job Summary:

Seeking a Social Worker to provide psychosocial assessments, care coordination, and support services for patients and families. The role focuses on advocacy, discharge planning, and connecting individuals with appropriate community and healthcare resources.

Roles & Responsibilities:

• Conduct psycho social assessments and identify patient needs and barriers to care.
• Develop and coordinate care and discharge plans with interdisciplinary teams.
• Provide counseling, crisis intervention, and emotional support to patients and families.
• Connect patients with community resources, benefits, and support services.
• Document assessments and interventions accurately in the EMR and ensure regulatory compliance.

Required Qualifications:

• LCSW (required)
• At least 2 years of experience.
• Case Management experience (required)
• Active BLS
